Congratulations on your admission for 2021. To confirm your enrollment, please submit all forms and documentation detailed below. All items except the deposit should be returned directly to Zicklin Executive Programs.
Required Enrollment Documentation
These items are required to confirm your seat in the cohort and to be eligible for course registration.
Enrollment Contract and Acceptance Deposit
The $2,500 ($2,750 for DPSB) non-refundable Acceptance Deposit and a signed copy of the Enrollment Contract should be returned at the same time. It is important to carefully read through the Enrollment Contract and note the due dates for your program fee installments. The Acceptance Deposit can be paid via wire transfer directly to the Baruch College Bursar (see email for instructions). Your seat in the cohort will not be confirmed until both of these items have been received.
The signed Enrollment Contract can be scanned and emailed to email@example.com.
If you have ever been enrolled in credit-bearing coursework at a CUNY school, you may not have to resubmit immunization documentation. Once you confirm your enrollment and we have created your student record, we can check to see if you have already satisfied the immunization requirement from your previous academic career.
For all other new students, please complete Part I of the form, sign, and date it. Your Health Care Provider must sign and stamp Part 2 of the form or the immunization records themselves, if submitting original records. Please return both the completed form and the immunization record together to firstname.lastname@example.org.
Students cannot be registered for classes until they have satisfied the immunization requirement. If you are unsure about your immunization history or are not able to obtain records, your doctor or local walk-in clinic may be able to administer a blood titre to determine immunity.

The Executive Biography will be shared with your cohort, faculty, and staff. Specific instructions and examples are enclosed. This item can be submitted as a Word document via email to email@example.com.
The Professional Headshot will accompany your Executive Biography. By “professional”, we mean a headshot that would be appropriate for a LinkedIn profile. We recommend using a photo with a light-colored background if possible. We prefer the photo to be 2″x 2″. Please submit an electronic file (in format such as .jpeg or .png) via email to firstname.lastname@example.org.
Sponsor Agreement (optional)
Especially if your employer will be financially sponsoring your program, we strongly advise completing this form with your supervisor or your HR department. You may return the completed form to the Student Services Team at email@example.com.
