I am referring to you Mr. Tamayo Belliard, an employee at The Hudson Companies, who has been complaining of a severe pain in his hip. A copy of Mr. Belliard’s medical record at our company is enclosed.
Please observe the following item from the patient’s medical records:
1. Mr. Jones sustained an impacted fracture of his hip when he was a child.
2. This injury has never caused him any work-related problems, but I suspect that arthritis may have developed.
Will you please take the necessary X-rays and inform me of your findings. Mr. Belliard carried group health insurance with our company; his policy will cover all medical costs associated with your services
